A trial of 1,800 German adolescents showed that cognitive behavioral therapy prevented gaming disorder far better than media education. This result had direct relevance for mental health treatment facilities and dual diagnosis treatment programs serving teens with co-occurring conditions.

Approaches in the Study

Problematic online habits like excessive social media use can erode mental and physical health during the teenage years.

The trial enrolled 1,793 teens aged 11 to 18 from 44 secondary schools in Baden-Württemberg. Half received PROTECTtraining, a cognitive behavioral therapy program covering the addiction cycle, behavior change skills and emotion regulation. The other half received PROTECTinfo, a media education program covering media literacy and cyberbullying.

Researchers measured online addiction symptoms and mental wellness at the start and end of the program and followed up after 4 and 12 months. Neither program showed a measurable effect a year later among those at low risk. Adolescents flagged as high risk benefitted, but the behavioral therapy program was clearly stronger. 

The therapy program proved about 50% more effective against gaming disorder and other problematic online behaviors. The researchers concluded that targeted programs for at-risk students outperform broader school-wide prevention efforts while using fewer resources .

Dual Diagnosis & Treatment Options

Co-occurring disorders describe a mental health condition and an addictive behavior happening in the same person at the same time, each reinforcing the other. A teen with depression may retreat into drugs or gaming for relief, which creates a cycle of dependence.

Teens with acute gaming addictions might consider behavioral inpatient or outpatient care

Evidence-based options include behavioral therapy to break the cycle behind compulsive use. Family counseling addresses the household patterns sustaining the behavior. When depression or anxiety strikes, medications prescribed by a psychiatrist can stabilize those symptoms so therapy can take hold.
