San Antonio Weighs How to Route Higher Risk Mental Health Calls

San Antonio is working through a question many cities face, which is how to route mental health calls that fall outside what a specialized crisis team is designed to handle.

The answer shapes what happens to people who need mental health treatment when a situation escalates faster than the system anticipates.

The question drew fresh attention after a fatal encounter. Just before midnight on August 16, 2026, officers responded to a reported disturbance in the 6700 block of Spring Front Drive on the Northwest Side.

According to SAPD acting chief Jesse Salame, a 56-year-old man had called 911 to report that people were in his backyard and that one was armed.

No one was found armed. Officers encountered the caller carrying two large knives, and after he refused commands to put them down and advanced toward an officer, the officer fired, striking him four times.

The department said the man may have been experiencing a mental episode. It was the eighth SAPD shooting of 2026.

Mental Health and Addiction Connection

San Antonio operates a specialized crisis response program. SA CORE, the San Antonio Community Outreach and Resiliency Effort, fields three teams, each made up of a mental health unit officer, a fire department paramedic, and a licensed clinician, providing 24-hour coverage across the city.

Those teams are dispatched to lower-acuity calls, meaning situations where no weapon is involved and the encounter has not escalated.

That threshold is a deliberate design choice built around what a clinician can safely do on scene. Calls falling outside it go to standard patrol officers, and that is the routing question the city is now examining.

The distinction matters for behavioral health because psychiatric crises and substance use frequently overlap, and the combination tends to produce exactly the kind of unpredictable, fast-moving situation that sits above the threshold.

Intoxication, withdrawal, and stimulant-related agitation can all mimic or intensify psychiatric symptoms, which makes accurate assessment in the field harder.

What San Antonio Is Building Toward

City officials have been developing a 911-level triage pilot with the Southwest Texas Regional Advisory Council and the Center for Health Care Services that could route low-acuity mental health calls to a clinician by phone, without dispatching police or emergency medical services.

In early August 2026, city Chief Mental Health Officer Jessie Higgins said that program was still in development and under review.

The city has expanded this capacity before. In June 2023, three SAPD officers were charged with murder in the shooting death of Melissa Perez, a woman then police chief William McManus said appeared to be having a mental health crisis.

McManus said at the time that the department’s mental health unit was available that night but was never requested.

In the months that followed, the city expanded SA CORE to 24-hour coverage. SAPD has not confirmed whether the man killed in August had a diagnosed mental health condition, and the investigation is ongoing.

Treatment Options for Mental Health and Addiction

For people whose needs sit at the intersection of psychiatric symptoms and substance use, several levels of care exist:

  • Residential treatment centers provide 24-hour structure and are often appropriate after a crisis, or when symptoms have not stabilized in outpatient care
  • Partial hospitalization and intensive outpatient programs offer substantial clinical hours while allowing people to live at home
  • Standard outpatient behavioral health combines therapy with psychiatric medication management
  • Crisis stabilization units provide short-term care as an alternative to emergency departments or jail

Evidence-based therapies used across these settings include cognitive behavioral therapy, dialectical behavior therapy, and trauma-focused approaches.
