Welcome to Southwest Behavioral Health Services Community Transition Program. Our team is dedicated to providing high-quality, evidence-based care for individuals with mental health and substance use disorders. We offer a variety of services, including dual diagnosis treatment, cognitive behavioral therapy, experiential therapy, group therapy, individual therapy, and trauma therapy. Our program is also LGBTQ friendly and military friendly. We provide care for adults, young adults, and seniors in our inpatient and outpatient rehab programs.
